Amniotic fluid (AF) cell disorders and chromosome abnormalities in AF cells are commonly seen in prenatal cytogenetic diagnosis, but are present only occasionally in normal AF cells. Unidentified abnormal cells in normal AF can lead to false positive diagnoses, whereas unidentified normal cells in abnormal AF can lead to false negative diagnoses. As yet, no studies have shown what the distribution of abnormality in AF cells is; nor the combination of chromosome abnormalities in AF cells, which can provide clues for prenatal diagnosis. We used a multi-color fluorescence in situ hybridization (FISH) with a whole chromosome painting (WCP) probe to assess the cell-type-specific frequencies of chromosomal abnormalities. We also assessed the efficacy of FISH for detecting sub-chromosomal changes in AF cells in AF cell disorders (AFCD). The karyotypes of both normal and abnormal AF samples from 511 pregnancies were characterized by G-banding analysis, and detected abnormal cells in 126 (25%) samples. In 22 (18.5%) samples with isolated abnormalities, the AF cells were normal. In 32 (25%) samples with a normal karyotype, an abnormal karyotype was found in 10 (30.3%), including four (12.5%) chromosomal aneuploidies, five (15.6%) balanced structural rearrangements, and one (3.1%) unbalanced translocation. In 20 (16.1%) samples, an unexpected cell-type-specific chromosomal abnormality was detected in only one of the other two cell types (AF cells and maternal lymphocytes) of the respective pregnancy. Our results provide evidence that the frequency of chromosome abnormalities in normal AF cells is significantly higher than that in AF cells. Chromosome 19 abnormalities in AF cells and chromosome 1 abnormalities in the maternal lymphocytes of the same pregnancy were found in 1.2% of normal AF samples. By FISH with WCP probes, chromosome abnormalities in AF cells of AFCD cases can be revealed in both normal and abnormal AF samples, and the combination of abnormalities in the AF cells of an AFCD case can provide some clues for prenatal diagnosis.Q
