objectives: this study reports the development of a virtual fixture that can be used to guide mandibular reconstruction, which can provide an accurate and convenient template for mandibular reconstruction.

methods: the basic fixture elements are divided into three essential categories: clamping elements, positioning and guiding elements, and supporting and base elements. a knowledge-based program structure is used to decide on the appropriate fixture with respect to a particular workpiece specification. the drawing of the elements is then parametrically constructed with an interface program calling standard commands from a drafting package. the elements are assembled interactively to form the final description of the fixture.

the workflow for the camrp system is as follows. the cadrp system: the cadrp model is built in the cad layer. the cam layer is generated from the cadrp model. the camrp layer is imported into the camrp environment. the camrp environment provides the location of the cutting plane points, virtual machining centre and virtual fixture points. the camrp environment is also capable of simulating the cutting conditions and facilitating the determination of virtual machining centre and virtual fixture location. the camrp environment can also help the doctor to adjust the cutting plane positions according to the virtual machining centre and virtual fixture locations.

to solve these problems, a new design system for mandible reconstruction using a virtual fixture is developed in this study. the virtual fixture is considered to be a guiding tool for the fibula and a fixture for positioning the mandible during mandible reconstruction surgery. it provides both an accurate and convenient virtual reconstruction method and can be designed rapidly using a computer-aided design (cad) program. this design method is based on the combination of cad, medical imaging, and surgical navigation technology. the cad software used in this study is a virtual fixture design software developed by an overseas company. it is able to analyze the shape of the mandible, simulate and predict the cutting of fibula, and provide a guide for the surgeons to ensure the satisfactory shape of mandible and fibula. this study compares the performances of the virtual fixture with a traditional method, in terms of the cutting accuracy and the degree of fibula shape, to evaluate the impact of the virtual fixture on mandible reconstruction surgery. a finite element model was developed based on the ct scans of a mandible. the mandible was then virtually reconstructed and the virtual fixtures were installed. the model was used to predict the fibula cutting line, which was compared with the fibula line designed by a traditional method. a virtual fixture was designed to guide the fibula cutting. the prediction result was used to reconstruct the mandible defect. the study also compared the differences in the degree of shape of the mandible and the fibula with the traditional method and the virtual fixture method. the results indicate that the virtual fixture method is an accurate and convenient method for designing and simulating the virtual fixture, and it can be used to reconstruct the mandible in the clinical setting with the same accuracy as the traditional method. 5ec8ef588b
