The Norwegian Breast Cancer Screening Registry was investigated for all 50-year-old women with nontoxic thyroid autoimmunity or overt hypothyroidism between 1971 and 2001. Data from the four largest central hospitals and the Cancer Registry of Norway were used to calculate incidence rates of breast cancer according to time, age, and cause of disease. The cohort consisted of 43,823 patients with nontoxic thyroid autoimmunity and 620 patients with overt hypothyroidism. The breast cancer incidence rate was 1.9 per 10,000 person-years for the autoimmune group and 10.7 per 10,000 person-years for the hypothyroid group. Among the patients with hypothyroidism, a significant increase in the breast cancer incidence rate could be confirmed, with the incidence rate rising from 3.4 per 10,000 person-years for patients diagnosed between 1976 and 1980, to 31.7 per 10,000 person-years for patients diagnosed between 1991 and 1995. A significant excess of patients with breast cancer was observed when the time periods 1976-1980 and 1991-1995 were compared (p = 0.0052). The breast cancer incidence rate was 9.4 per 10,000 person-years among those with the most severe thyroid dysfunction (TSH, < 0.02 mIU/L), who represented 10.3% of the hypothyroid patients. The present study shows that nontoxic thyroid autoimmunity and overt hypothyroidism constitute an important risk factor for breast cancer development.
