The basics: Tools and paths Photoshop provides a wide array of tools for manipulating image data. Many of these tools exist in a menu or toolbar at the top of your screen, and they work in layers. You can move or delete a layer through the Layers Panel window to remove or create a new layer. If the original image is visible, it will be visible and editable. The right side of the Layers Panel window displays a list of layer attributes, from top to bottom: Selection Color, Blend Mode, Erase Color, Opacity, and the Layer Level. Where to get Photoshop? You can get it from Adobe through online retailer affiliate PartnerWise, along with newer models. Or you can pay full price for the program, which will likely be cheaper than buying the application at an electronics store. Adobe charges $119 for the standard version and $299 for a pro version. Using Photoshop Elements is a more affordable way to get Photoshop, with a subscription fee ranging from $10 to $40 per month. The next layer: A selection tool The most common tools in the Toolbox are the Rectangular and Elliptical Marquee tools. These tools are extremely useful for creating selections for using in Photoshop. You can navigate your canvas through the Selection Tool. Holding down the SHIFT key while dragging the selection tool enables you to create a perfect round or square area that can contain multiple objects. The Make Selection or Rectangular Selection Tool The Selection Options window enables you to change the brush, brush size, and other important settings. You can read about this tool in the next section. The next layer: An eraser The only tool in Photoshop that will erase the contents of a layer is the Eraser tool. Using the Eraser, you can draw with a line to destroy or remove parts of an image. You can also erase areas of pixels with the Magic Wand tool, but it takes a lot of practice to make it work as you'd like. An example of an advanced eraser Using the Eraser is a completely different experience than using a pencil or other tools. This tool erases the contents in its path, which can make it easier to draw over an area of an image and safely remove it when you no longer want to use it. Due to its small amount of features, it is one of the most common programs Photoshop users use in conjunction with other professional software such as Adobe Lightroom (formerly known as Adobe Photoshop Lightroom) or Adobe Camera Raw (formerly known as Adobe Photoshop Camera Raw). Some graphic designers choose to use Photoshop Elements for its simplicity and speed. Adobe Photoshop Elements for Windows (Mac is included as part of the Creative Cloud as of 2018) If you wish to use Photoshop and have enough resources, you can get Adobe Photoshop Elements 16, which is the last version with free updates and is 10 years old. It was a big success and still sells, although its sales have decreased (according to Adobe's results for the financial quarter ending June 30, 2017). If you wish to get all the features of the full version of Photoshop, it is necessary to get a subscription to Adobe's Creative Cloud, which costs $99 per year. There is also a free 30-day trial for those who wish to test this software. You can save files to your computer hard disk, a network drive or an online cloud storage service such as Dropbox. You can also send the image files directly by e-mail using the desktop version of the application. For those who use Adobe Photoshop Elements as a cloud storage solution, it is only necessary to have a valid Adobe ID. For those who use the desktop version of the program, it is possible to keep the original files in the cloud and have access to them from your computer, a tablet or smartphone. Adobe Photoshop Elements for Mac Adobe Photoshop Elements has a simple menu, the top left corner menu (Windows only) or F3 function in the Mac version (if you press the green flag at the left side of the menu bar). Additives menu The image correction options in Photoshop Elements are located in the Edit > Additives menu. This menu allows you to correct image properties such as contrast, color, exposure, white balance, saturation, gamma and levels. Effects Effects are a type of filter that can be applied to photos, text, or layers. They can be used to create cool effects like fizz, swirl, blur, and glass. They are also used to create a variety of other things like bevels, patterns, textures, drop shadows, perspective and more. Some popular effects include Glare, Grain, Gradient, Tile, Hue-Saturation, Drop Shadow, and Glow. You can access the effects that you have installed in Photoshop under Edit > Effects > Develop Module > Filter Gallery. 3. Textures Textures are interesting because you can use them to create distinctive visual effects that are often very difficult to produce in real life. For example, you can use textures to set a watercolor-type look on your textures. You can use any kind of image or shape to be a texture. Photoshop CS4 includes two new options, known as a Grab and Mask. You can also make your own textures. You can use this method to make new ones or modify an existing one. The process for making new ones is described in detail in Make a new custom brush in Photoshop. 4. Shadows Shadows are probably one of the most recognizable and useful Photoshop features. You can create a light or dark shadow by choosing a blur tool (like the Blur Tool or Gaussian Blur Tool) or making a selection and using the Shadow or Burn tools. The two most common types of shadows are Single Shadow and Multiple Shadow. To make a Single Shadow, select a part of your image and blur the edges with the blur tool. To make a Multiple Shadow, first create a selection that defines the areas where you want your shadow to be created. Then use the Shadow or Burn tools to create multiple shadows. You can use the layer mask to easily blend the effects into your image. 5. Thresholds and Masks Thresholds and Masks are used to make selections of specific areas of an image. You can use the Threshold tool for non-destructive selections or the Threshold Mask tool for destructive selections. The Threshold tool
CASE REPORT =========== A 54-year-old woman was referred for acute chylous pericardial effusion. She had been diagnosed with breast cancer and had undergone a simple mastectomy in the right breast. The tumor had a 4 cm maximum diameter, was estrogen and progesterone receptor positive, and was well differentiated. After 3 years of follow-up, she had no recurrence and had had no further treatment until 1 week before admission. At that time, she started to complain of mild upper abdominal discomfort. The initial laboratory studies performed at an outside hospital were normal, but the patient was lost to follow-up after the 1-week visit. She had no history of diuretic use or medication use but had a 10-pack-year history of smoking. There was no history of trauma or surgery. In a physical examination, blood pressure was 120/78 mmHg, heart rate was 80 beats per minute, and her respiratory rate was 20 breaths per minute. In a cardiac examination, a distended abdomen was palpated with no jugular venous distension. A grade 4/6 systolic murmur was heard in the right upper lung field. Jugular venous distension was also observed. Transthoracic echocardiography revealed a circumferential, soft, highly echogenic, cord-like mass (52 × 46 × 44 mm) in the right atrium that showed a'mustard seed-like' appearance by displaying a hyperechoic central area (Figure [1](#F1){ref-type="fig"}). It was attached to the anterior wall of the right atrium and caused a slight right atrial collapse. Color Doppler showed massive flow of the thoracic duct. There was no abnormality of the atrial cavity and valves. The left atrium and ventricle were also normal. The patient was diagnosed with mediastinal chylous pericardial effusion and subclavian venous compression. After the patient was placed in the Trendelenburg position, the mass disappeared (Figure [2](#F2){ref-type="fig"}). The chylous pericardial effusion was drained with an insertion of a pericardial catheter. After drainage, the pericardial fluid was milky and contained about 600 cc of chylomicrons.
